Comment: I know this remains incomplete but FertAid wishes to conduct an annual census of IVF Laboratory activities. Many questions will not apply but please complete all questions. One reply per lab please. Please circulate as widely as possible to deliver the best census data.

#0 writes: Some small issues: 1. Oocyte donation (only really possible in certain countries 2. Some questions (eg 21/22) are ambiguous. We offer and perform assisted hatching, but not as a routine on every case.

#0 writes: Curious about programs doing all cryo for elevated progesterone levels on day of trigger or post-trigger day. Also, if TE biopsy is being performed on thawed embryos, if they are re-freezing or rushing results for "D6" ET. Sometimes difficult to answer just yes or no for some questions posed.
#0 writes: reason for women <35 yrs to seek ART is usually male infertility, thus ICSI. To ask about ICSI above 35 yrs would be a more informative question to see whether clinics advise more ICSI then necessary.
